Bring your medical records (within the last year), current medication bottles/prescriptions, or any medical history that can be used as verification of your qualifying condition.

Day of your appointment: make sure to bring your photo ID, MMCC patient ID number, medical records/pill bottles and any records to verify your qualifying condition. Our knowledgeable provider will assess you, review your medical records, educate you on the risks and benefits of medical cannabis, answer any questions you might have and if qualified, you will receive your written Medical Marijuana Certification at the end of your visit.
