WHAT TO EXPECT:
This is an evaluation to assess the need for an alternative treatment for patients who meets the criteria for qualifying conditions in the state of Maryland. These conditions include chronic or debilitating disease or medical condition that causes: cachexia, anorexia, wasting syndrome, severe or chronic pain, severe nausea, seizures, severe or persistent muscle spasms, glaucoma, post-traumatic stress disorder (PTSD), or another chronic medical condition which is severe and for which other treatments have been ineffective.
PRE-VISIT REQUIREMENTS:
A required physical exam prior to operating a commercial vehicle. Also referred as a “medical fitness for duty” examination, to ensure that each driver is capable and healthy enough to successfully complete the duties of a commercial driver. Our Medical Examiner follows the health standards are determined by the FMCSA. In order to comply with federal law, your DOT physical exam form must be completed by a qualified Medical Examiner.
